Who this is for: The assigned auditor.
On the clause row, select +Findings — the button at the right-hand end of the row.
The Add Finding pop-up opens with the Clause Reference already filled in for you.
Complete the fields. They are listed in the table below.
Continue through the next steps — Root Cause and Corrective Action — or save the finding and come back to them later, if your template allows.
Field | Required | Notes |
Finding Type | Yes | Nonconformity, Observation, Opportunity for Improvement or Positive Finding |
Severity | Yes for nonconformities | Major or Minor |
Statement | Yes | What was found |
Objective Evidence | Depends | Required where the template or clause requires evidence |
Photo | Depends | Optional or required depending on the template or clause |
Auditor’s Notes | No | Context, commentary, what you discussed |
Clause Reference | Yes | Filled in when raised from a clause |
A finding card appears beneath the clause row.
Use Positive Finding to record something done notably well — see 0.3.
Every finding raised inside an audit must be linked to a clause. If you raise a finding from the Findings step instead, you must choose the clause yourself. No answer raises a finding on its own: every finding is raised by pressing +Findings.
