Who this is for: The assigned auditor.
This is for problems with running the audit, not problems with what you are auditing — access refused, a safety concern, obstruction, or information you need and cannot get.
A nonconformity is a finding about the auditee’s compliance. A raised issue is a problem doing the audit at all. They are handled completely differently — an issue produces no finding and no corrective action, only an escalation.
Select Issues in the Audit Overview panel on the right of the audit wizard. The badge shows how many issues are open.
Select Raise Issue.
Complete the pop-up. The fields are listed in the table below.
Select Raise Issue.
Field | Required | Notes |
Issue type | Yes | Access denied, Safety concern, Obstruction, Missing information or Other |
Specify | Yes, for Other | A short label that is shown in place of “Other” |
Description | Yes | What is stopping the audit |
Files | No | A photograph or document, for example a locked door |
What happens next:
The audit moves to Blocked.
The audit owner and any subscribers are notified.
An entry is written to the audit’s history log.
You can keep working on the audit, but it cannot be published while it is Blocked.
The audit owner resolves the issue from the Issues panel, recording a resolution note. When every open issue is resolved, the audit returns to In Progress. See 11.5.
Declaring a conflict of interest also raises an issue of this kind automatically.
