Adding a root cause analysis after the audit

Running the analysis when the auditor did not.

Who this is for: Needs Manage Root Cause.

Auditors do not always run the root cause analysis during the audit — often deliberately, so they can move quickly on site. It can be added afterwards, and it does not have to be the auditor who does it. Many organisations have an operations or quality team do this work.

  1. Open the finding from Audit Findings.

  2. Select Conduct Root Cause Analysis.

  3. Choose a method — 5 Whys, Fishbone, Fault Tree Analysis, or Skip.

  4. Work through the method’s fields.

  5. Choose a Root Cause Category — this is required whichever method you use.

  6. Optionally add a Root Cause Summary in your own words.

  7. Save.

After saving, the button changes to View Root Cause Analysis with a purple tint, and the chosen category appears as a purple pill on the finding.

Which methods are available is set by your administrator, and is read live at the moment you run the analysis — so if the available methods changed after the finding was raised, you see the current set, not the old one.
