Managing root cause categories

Configuring the categories auditors choose from.

Written by Caim Hermoso

Who this is for: Audit Administrators.

In Settings → System Settings → Audit Settings → Records & Findings, the Root Cause Categories table controls the dropdown auditors see when recording a root cause.

Each row has:

  • Name — editable inline

  • Group — Management & Leadership, Process & Procedure, People & Training, Equipment & Environment, Monitoring & Review, or Other

  • Enable/Disable — disabled categories are greyed and excluded from the dropdown

  • Delete — removes the row. You cannot delete the last remaining category

Add Category appends a new editable row.

Fourteen categories are pre-seeded, covering the six groups.

Disable rather than delete. Deleting a category that has been used breaks the history behind your root cause trend report. Disabling it removes it from the dropdown for future findings while leaving past findings intact.

Keep the list short. Fourteen categories already produces a meaningful trend report. Thirty produces a list nobody reads carefully, and auditors picking whatever is at the top.
