The Witnesses Tab

The Witness information tab appears on the Incident/Accident record type.

It is used to record all witnesses that saw the event or incident take place.

To add a new witness to a record, start by clicking the Add Witness button.

The Add New Witness window will appear. Simply add in the following witness details outlined below:

  • First Name - The witness first name

  • Last Name - The last name of the witness

  • Business Unit - The primary location of the witness

  • Location - The witness's location below the Business Unit 

  • Employment Status - State whether the witness is an Employee, Contractor, Member of public, or Other

  • Job Title - The job title of the witness

  • Email - The witness's email address

  • Phone - The phone number of the witness

When all data has been added, click the Add button to add the witness to the record. 

Please Note: Certain information is required in order to add the witness to the record. If you try clicking the Add button and some data is missing, you will be prompted and all data fields that require information before moving on will be highlighted in red:

Once completed, you will see the new witness added to the list of witnesses:

Rays Advice - You can Edit or Delete an witness' information by clicking the relative icon. See below:

Adding Witness Statements

Once you have added a witness to a record, you may want to add a witness statement document.

To do this, first click the Upload icon next to the witness. See below:

On the Add New Attachment window that pops up, click the Browse button and attach the witness statement from your computer:

Once added, you will notice that the Upload icon has changed to a Download icon. This has been highlighted below. Simply click the Download icon to download an existing statement for that witness.

Rays Advice - You can also download all witness statements via the record's Attachment section. The witness statements will not appear in the main list of attachments, but if you click the Download Attachments button, you will see a list of all attachments and all witness statements. You can then download any attachment or witness document by clicking it's Download Attachment icon. Please see the two screenshots below:

There will also be a new Upload icon that can be used to upload a new witness statement if one already exists. This can be seen below:

To replace a witness statement, simply click the Upload icon, and then click the Browse button to locate the replacement document file.
