The Activity Info tab appears in the Incident/Accident record and is used to record the details of the task or activity being carried out. 

An overview of the information to be added to the Activity Info tab can be seen below:

  • Identify activity undertaken at time of incident - Describe the work or action that was being carried out

  • Is a risk assessment available for this activity or area? - State if a risk assessment was available (Yes, No). If answered with Yes, a new question Does the risk assessment need to be revised? will appear next to it. (Yes, No). See below:

  • Was a Permit To Work issued to undertake this activity? - State if a permit-to-work was issued to authorise to carry out specific work within a specified time frame (Yes, No, N/A).

  • Was the affected person authorised to undertake this activity? - State if the affected person was authorised to carry out the work (Yes, No, N/A).

  • Has the affected person been trained and competent for the activity? - State if the affected person has had training to carry out the work (Yes, No, N/A).

  • At the time of the incident, was PPE required? - State if Personal Protective Equipment (PPE) was required whilst carrying out the work.

  • What were the environmental conditions like at the time of the incident? - Describe the setting/surrounding conditions at the time the of the incident.

When all data has been added, click the Next button to continue to the next tab. 

Please Note: Certain information is required in order to continue to the next tab in the record. If you try clicking the Next button and some data is missing on the current tab, you will be prompted and all data fields that require information before moving on will be highlighted in red:
