The Assessment Details section is the first step of the assessment wizard process, and consists of the following elements:
Task Title – This is the title task being assessed.
Process Title – This is an optional section that can be used for additional information
Location – This is where the assessment is set at in the company structure
Assessed On – This is unchangeable, and holds the date the user logged in
Assessed By – This initially has the name of the person who began the assessment. If someone else edits/publishes the assessment, their name will be appended to names already present
Review Date – The system will monitor the date set here to determine if the assessment is in date or overdue and requires re-publishing
Reference – This is the unique reference identifier of the assessment
Signed By – This is unchangeable, and will contain the name of the user logged in when the assessment is published
Revision – This will show a value once the assessment is published for the first time, and will subsequently increment by 1 each time the assessment is re-published
People Affected – A list of people who could be affected by the task being assessed (Custom entries not in the list can be added by typing them in manually)
If the Super User of your system has pre-defined additional logos for you to use in your assessments, you can change the assessment's Logo by simply clicking the logo itself and selecting an alternative logo image.
Click the Save and Next button in the assessment wizard to save the information added and continue to the next step.
Our Advice: The Before Controls and After Controls displayed in the Assessment Details will automatically calculate once all Task Hazards have been assessed and rated (step 4 of the assessment wizard). These calculations will show the worst case scenario of all hazard before control ratings, and all hazard after control ratings respectively.
