③ Checklist Columns

The columns the auditor fills in against each clause, and how you choose them.

Who this is for: Needs Manage My Audit Templates.

Columns are set once for the whole template, not clause by clause. Every clause in the questionnaire then carries the same set of controls. You choose them in ③ Checklist Columns, which has two halves.

RESPONSE / ASSESSMENT

Column

What the auditor sees on each clause

Compliant / Non-Compliant

C and NC radio buttons

Areas Assessed

Tick boxes recording which areas were covered for that clause

Score

A score box and the maximum, for example "/5". Set the maximum with Maximum score per clause — the default is 5

Risk Rating

A Set button on each clause. It opens a 5 × 5 Risk Matrix of Severity of Harm against Likelihood of Harm, and the chosen cell shows on the clause as, for example, 16/25 · High

FINDINGS (PER CLAUSE)

Two further columns add supporting detail rather than a judgement:

Column

What it adds

Observation

A place to record what was seen on the clause itself

Evidence / Notes

An inline field for recording evidence as you go. This is also where the template's evidence requirements are configured — see 3.13

This section is also where the template states how findings get raised: "Findings are raised while the audit is conducted — every clause row carries a +Findings button." What the auditor must then fill in is set in ④ Findings Fields — see 3.12.

Choosing between them

Compliant / Non-Compliant is right for most compliance clauses — a requirement is either met or it is not. Add Score where you want a measurable result and a comparison between audits of the same area. Add Risk Rating where the consequence of a gap varies clause by clause and you want the report to show that. Areas Assessed records coverage rather than outcome, which is what answers the question how thorough was this audit.

Use the full set sparingly. Four response columns on a 90-clause questionnaire is a long day for the auditor and a crowded report for the reader.

One dependency to know about. If you want the Audit Summary to carry scores, the Score column must be on here. Switch Include Audit Summary on in ⑥ while Score is off and the builder warns you — see 3.14.
