Recording responses and marking areas assessed

Filling in the clause columns.

Written by Caim Hermoso

Who this is for: The assigned auditor.

Which columns you see depends on the template. The full set:

Compliant / Non-Compliant — a straight binary judgement on the clause.

Areas Assessed — tick boxes recording which areas you actually covered for that clause. These do not indicate a pass or fail; they record scope of coverage, which matters when someone later asks how thorough the audit was.

Evidence / Notes — a free-text field on the row itself, for recording what you saw as you go rather than only when raising a finding.

Score — a numeric score for the clause, against the maximum the template sets.

Risk Rating — a Set button that opens the 5 × 5 Risk Matrix for that clause.
