The Hazard record type allows you to record any source of potential damage, harm or adverse health effects on something or someone.
The first page of a Hazard record is the Incident Details tab:
The initial page of the Incident Details tab requires the following data to be added:
Business Unit - The primary location that the hazard was based at
Location - The location below the Business Unit that the hazard was based at
Hazard Date - The date the hazard was acknowledged
Describe where the hazard is - Outline exactly where the hazard was spotted, for example, 'Outside the director's office entrance door, opposite the water machine.'
Describe the hazard - Explain the hazard in as much detail as possible to highlight the issue. For example, 'A delivery of A4 paper was left on the floor outside the director's office. This could potentially cause someone to trip.'
Suggested solutions - Can a solution to the hazard be suggested? If so, add it here. For example, 'Move the box of A4 paper to a store room or cupboard'.
Who reported the Hazard? - The name of the person that reported the hazard.
Line Manager's Name - The name of the Line Manager.
Line Manager's Email - The email of the Line Manager.
When all data has been added, click the Save and Next button to continue to the next tab.
Please Note: Certain information is required in order to continue to the next tab in the record. If you try clicking the Save and Next button and some data is missing on the current tab, you will be prompted and all data fields that require information before moving on will be highlighted in red:
Our Advice - If you do not have permission to continue adding information to a record, once you have clicked Save and Next, you will be prompted with a reference number notification similar to the one below:
